PARTY CABOODLE – LIABILITY WAIVER & RELEASE FORM

ACKNOWLEDGMENT, WAIVER, AND RELEASE OF LIABILITY

I, the undersigned parent or legal guardian of the above-named child, hereby give my full consent for my child to participate in the birthday party activities hosted by Party Caboodle aboard the Party Caboodle  stationary party bus.

I acknowledge and agree to the following:

• The Party Bus remains stationary during all events.

• Activities may include, but are not limited to: dancing, karaoke, crafts, dress-up, makeup, and games.

•  There are inherent risks associated with participation, including minor injuries such as trips or falls.

•  Party Caboodle and its staff are not liable for injury, loss, or damage except in cases of gross negligence or willful misconduct.

• I am responsible for notifying Party Caboodle of any medical conditions, allergies, or special needs my child has prior to participation.

MEDICAL AUTHORIZATION

In the event of a medical emergency, I authorize Party Caboodle staff to secure emergency medical care for my child and accept full financial responsibility for any associated costs.

 COVID-19 DISCLAIMER

I acknowledge that despite all reasonable efforts to sanitize and maintain a safe environment, Party Caboodle cannot guarantee a COVID-19-free environment. By signing this form, I accept the risk of potential exposure and will not hold Party Caboodle liable for any illness that may arise.

CANCELLATION & REFUND POLICY

If a child or group becomes disorderly and cannot be reasonably managed, Party Caboodle reserves the right to stop or cancel the party at any time. This is at the sole discretion of Party Caboodle staff and intended to maintain a safe and enjoyable experience for all.
